"To err is human" but to do it over and over again makes you a damned fool.

There are some types of thinking errors that are so common they have been given cute names; and they are so costly to businesses that they have been analyzed and written in the journals. 

A colleague of mine, Omar Mahmoud, has summarized these in a terrific way, in a single document.  There's a lot of content, so it will take time to get through it all.  Take a piece at a time-- and remember it.  Immunize yourself.
